There is a hidden stat for happiness.
The stat needs to be maxed out for pokemon to evolve. 255 is max.
Your Poketch will tell you what range your pokemon is in.
No hearts, happiness = 0-99
One heart, happiness = 100-199
Two hearts, happiness = 200-255
WARNING: letting a pokemon faint in battle or trading a pokemon, resets the happiness value to 0!
Also: If a pokemon is critically wounded [beeping sound] or has a bad status [sleep, poison,...], it will not receive its happiness points.
Holding Soothe Bell increases points 50% of what is listed below.
Walk 256 steps with the Pokemon in your active Party.
(+1 point)
Feeding pokemon negative EV berry.
[berries 21-26, pomeg - tamato]
(+10 points)
Grooming your pokemon by massage.
(+3 points if its happiness stat is 0-199,
+1 point if its happiness stat is 200-254)
Give the Pokemon Vitamins.
[HP Up, Carbos, Iron, Protein, Calcium, Zinc, PP Up]
(+5 points if its happiness stat is 0-99,
+3 points if its happiness stat is 100-199,
+2 points if its happiness stat is 200-254)
Usage in Important Battle.
[Gym Leaders, Elites, Champion]
(+3 points if its happiness stat is 0-99,
+2 points if its happiness stat is 100-199,
+1 point if its happiness stat is 200-254)
Level Up the Pokemon.
(+5 points if its happiness stat is 0-99,
+3 points if its happiness stat is 100-199,
+2 points if its happiness stat is 200-254)

If for some reason you are an evil trainer and want unhappy pokemon.
The attack Frustration benefits from an unhappy pokemon.
Where as Return benefits from a happy pokemon with a high happiness stat.
You can decrease the happiness stat using the following items.
Energypowder
(-5 point if its happiness stat is 0-199,
-10 point if its happiness stat is 200-249)
Heal Powder
(-5 point if its happiness stat is 0-199,
-10 point if its happiness stat is 200-249)
Energy Root
(-10 point if its happiness stat is 0-199,
-15 point if its happiness stat is 200-249)
Revival Herb
(-15 point if its happiness stat is 0-199,
-20 point if its happiness stat is 200-249)
Edit 1:
Some people debate that letting a pokemon faint only gives -1 point to the happiness stat. My experimentation has shown its a happiness reset, but I'm only human...I could be wrong. My advice is to not take the chance if you are trying to evolve a pokemon.

Pokemon have a base happiness stat as well.
It is the happiness value it will have when its hatched, caught or traded.
